Plastic Bag Recycler Information Form

We are designing an online form for use by recyclers that accept one or more varieties of plastic bags. Until that form is available, please send us a direct email with information about your plastic bag recycling program.

So that we can make the information most useful to the public, please provide us with the following information as part of your email:

*The name and physical location where the public should take their plastic bags;

*The contact information for the consuming public (e.g., phone number, email, etc.);

*What kind of plastic bags are acceptable for your recycling program (e.g., grocery bags, dry cleaning bags, etc.)

*Any special instructions or information that the public should be aware of (e.g., hours of operation, limitations, etc.)
